POSITION: Substance Abuse Clinical Counselor

 

REPORTS TO: Director of Services and Clinical Director (primary)

 

SUPERVISES: NO SUBORDINATE

 

PURPOSE:  Under supervision, to perform senior level professional work involving the delivery of mental health services to individuals in need of counseling and/or case management services; may supervise family advocate when required. 

To perform other support functions for a variety of ProActive Management programs; and to provide information and assistance to the staff and public regarding the assigned programs.

 

CLINICAL COUNSELOR RELATED DUTIES AND RESPONSIBILITES:

 

Responsibilities:

Supervision/staff development, training and education:

Assist in development, coordination and implementation of orientation and training methods.

Assist in the supervision of staff through meetings, in-service training’s, guest speakers and reviewing literature.

Review treatment plans.

Other related duties as determined by supervisor.

 

Patient care:

Provide counseling services and performs the responsibilities of a case manager as determined by supervisor.

Respond, as needed, to patient grievances and complaints.

Assist in the role of Hearing Officer reviewing termination appeals.

Assist in managing the day to day patient needs issues that may arise.

Other related duties as determined by supervisor.

 

Administrative:

Assist in the direction and supervision of intake operations.

Assist in quality assurance chart reviews, assessing clinical and medical documentation to ensure compliance with regulation and program standards.

Assist in implementation of Quality Improvement Program, initiatives and goals.

Assist with on call coverage.

Assist in annual reviews as determined by supervisor.

Other related duties as determined by supervisor.

 

Position Tasks:           

           

1. Clinical Responsibilities:

2. Performs substance abuse and psychosocial assessments

3. Works with treatment team in staff meetings to formulate the diagnosis, Diagnostic Summary, Treatment Plan, Discharge Plan, etc.

4. Plans, organizes, and facilitates group therapies

5. Conducts individual and family therapy as needed to enhance the patients’ treatment experience

6. Documents the patients’ treatment experience according to regulatory(   ) agency requirements

7. Assists in facilitating communication of clinical information, allowing for effective utilization of patients’ resources, continuity of care and effective discharge planning
Programmatic Responsibilities:   

8. Participates actively in program development and improvement (PI)

9. Participates in efforts to monitor program effectiveness

10. Assists with initiatives designed to improve compliance with regulatory agency requirements
