POSITION:    FAMILY ADVOCATE

 

 

REPORTS TO:   Team Leader and Utilization Manager

 

WORK SCHEDULE:  Flexible Sunday – Saturday    9:00 a.m. to 7:00 p.m.  (Depending on family’s schedule) 

 

 

 

PURPOSE:  Under general supervision, to perform case management work to facilitate the treatment plan process for clients referred for services. Perform other duties as required. . The primary role of your job is to provide services based on medical necessity, consistent with clinical recommendation and client needs; considering the vulnerability of our client population, you are required to provide the necessary intensive CSI/Family Skills training at least initially to mitigate multiple client needs and increase positive treatment outcomes. Any deviation from providing medically appropriate intensive services will be considered negligence and could lead to disciplinary action and/or termination of contract.  

 

 

 

 

DUTIES AND RESPONSIBILITES:

  • Provide behavioral or skill building interventions that are identified on clients’ individual service plan; you are required to practice within the scope of the position and will not provide therapeutic/counseling services.

  • Provide written documentation of all interactions with consumers.

  • Maintain appropriate documentation of all client contacts, contact with service providers, and any attempted contacts with clients, service providers, agencies and organizations.

  • Check e-mail daily and respond to requests within 24 hours from UM, TL and PMC administrative staff.

 

  • Perform and submit timely required documents, including:

  • Assist clinicians and team leads in completing the Basis 24 at midpoint 90 days from the beginning of treatment and at discharge for all clients under your care

  • Assist team leads and clinicians in completing the Perception of Care survey at discharge

  • All clients under your care will require an electronic discharge summaries no matter what the circumstances of termination of services; you have 30 days to complete the discharge summary from your last service date

  • referral source progress reports (bi-monthly),

  • You are required to provide necessary information to the authorization specialist if you are currently servicing CMO clients when there are 12 or less units of CSI/Family Skills Training remaining

  • Your are required to put you BIRP notes within 48 hours of the services date for all clients; the note needs to be signed locked 7 days from the day of service. All notes that do not comply with this guideline will be paid half of your contractual rate.

 

  • You are required to familiarize yourself with information on client’s medical records for effective services:

  • Assessment and treatment recommendation

  • Reading psychological reports and psychiatric assessments when available

  • Refers clients to appropriate social service agencies when needed

 

  • Participate in various meetings, staffing, in-service and training as required.  Mandatory attendance for PMC bi-monthly team meetings and annual training is required.

  • Establish and maintain effective public and working relationships and collateral contacts.

  • Complies with all State and Federal Program requirements.

  • Participate in annual performance appraisal.

  • Performs other duties as required.

QUALIFICATIONS:

 

 

Essential Knowledge and Abilities

  • Ability to use effective time management and organizational skills;

  • Ability to communicate effectively, both orally and in writing;

  • Working knowledge of computerized program software, Internet access and word processing programs;

  • Ability to establish and maintain effective working relationships with clients, families, service providers, colleagues and the general public;

  • Considerable knowledge of human services and health resources within an identified community;

  • Ability to maintain confidentiality and take prudent measures to protect and secure documents and records.

  •  

 

Training and Experience

  • Preferably a Bachelor’s degree from a recognized college or university;

  • Minimum of 1 year of professional experience with communicating with service providers, agencies, and municipal personnel;

  • Minimum of 3 years of working with clients that are diagnosed mental health disorders
